Culturally Centered Housing Stabilization Services

Housing Stabilization Services (HSS) is a Medical Assistance benefit to help people with disabilities and seniors find and keep housing. Parent Support Outreach Program

Shakir Consulting is a new PSOP provider with Ramsey County. Ramsey County is committed to supporting healthy families and strong communities. Our Parent Support Outreach Program (PSOP) can help connect you to the services and resources your family needs and work with you on support options to help your family thrive.

Case management assists the elderly, individuals with disabilities, and individuals with mental health conditions receive access to resources, supports and services to support them in living, working, and going to school in their communities. We work with individuals using a racial equity and culturally responsive lens. 

 

Shakir Consulting provides the following case management services to people of all ages and backgrounds in Ramsey County:

We provide Business Consulting, Organization Development and Change Management, and Racial Equity Training.

Basic Support Services

  • Individual Community Living Support (ICLS):  For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes.

  • Homemaker:  Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.

    • Cleaning:  Light housekeeping tasks and laundry

    • Home Management:  Homemaker cleaning, and arranging transportation, preparing meals, shopping, simple household repairs

    • Homemaker Assistance with ADLs:  Cleaning and home management, includes assistance with ambulating, bathing, dressing, eating, grooming, toileting.

  • Night Supervision:  Provides overnight assistance and monitoring by an awake staff in the person's home.

  • Individual Home Support (IHS) without Training:  This service option is provided to adults or children when they need support in at least one of the community living service categories.

  • Companion Services:  Non-medical care, supervision and socialization to a person age 18 or older.

  • Respite Care:  Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

  • 24-hour Emergency Assistance:  On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency.

 

Intensive Services and Supports

  • Individual Home Support (IHS) with Training 1:1:  This service option is provided to adults when they need support and training in at least one of the community living service categories.

  • Individual Home Support (IHS) with Family Training:  This service option is designed for people who live with their family to provide training to the person and their family members. The training increases their capabilities to care for and maintain the person’s ability to live in the home.
